Clinical studies of cefpiramide (Cefirad^(¢ç)) in the field of surgery

Abstract
Cefpiramide(Cefirad¢ç CPM), a newly semisynthetic cephalosporin, was investigated on its serum, bile and urine levels on 6 patients who were received T-tude choledochostomy and clinical effectiveness and side effects on 31 surgical patients who admitted at Department of Surgery, Kangnam St. Mary¢¥s Hospital. Catholic University Medical College, from January to June, 1990.
The results were as follows
1. Following intravenous one shot injection of 1g of CPM on 6 patients, the average serum level reached its peak of 209.06 ¡¾ 43.92?g /ml at 1 hour and gradually fell to 70. 10¡¾ 9,13?g /ml, 35.80 ¡¾ 25.83?g /ml and 17.13 ¡¾ 12.61? g /ml at 6 hour, 12 hour and 24 hour respectively. Urine and bile levels showed no definite distribution, but reached its peak of more than 500pg /ml respectively, and maintained relatively high levels of 266.00¡¾22.68?g /ml and 112.95 ¡¾ 81.85?g /ml at 24 hour respectively.
2. Overall clinical effectiveness of CPM was 83.3% (20 /24) in therapeutic use ; 90% (9/10) in peritonitis, 77.8%(7/9) in biliary infection and 80%(4/5) in miscellaneous infections. In case of prophylactic use, CPM was effective in 85.7%(6 /7).
3. According to daily dosage, CPM was effective in 80%(8/10) with the dose of Ig and 85.7%(12/14) in 2g.
4. In vitro sensitivity test, CPM was highly sensitive to gram positive bacteria and Serratia group (100%), E. coli (90%) and Enterobacter group and Klebsiella group (80% respectively). On the other hand, sensitivity to Pseudomonas aeruginosa and Citrobacter group was relatively low as 57.1% and 50% respectively. Sensitivity to anaerobic Bacteroides group was 75%.
5. No side effects were noted except one case of nausea and vomiting after administration of CPM which was regressed spontaneously. Elevation of SGOT and SGPT was noted in one case respectively but mild and reversible. Elevations of serum alkaline phosphatase were noted in four cases which were recovered in following one week.
